Academic Misconduct Incident Form


Please use this secure form to report alleged student academic dishonesty to the Office of Student Conduct. We take every report seriously and assess the possibility of discipline and sanctions under the University’s Student Conduct Code or other policies/laws that govern student, faculty, and staff conduct. Due to privacy laws, you may not learn the outcome of your complaint.
