Purpose: The Alzheimer's Association Public Health Center of Excellence on Dementia Risk Reduction, Bridge Builders Strategies (BBS) and the National Association of City and County Health Officials (NACCHO) are seeking to partner with at least two local public health departments to host Community Convenings on dementia risk reduction. The Community Convenings program is a structured process that brings together community leaders to address risk factors for dementia through cross-sector collaboration. These Community Convenings are designed to:
- Identify the community’s priorities,
- Discuss the risk factors for dementia, including the social determinants of health,
- Help develop community partnerships and joint actions to address these dementia risk factors.
As a Community Convenings host, your role will be to mobilize community members to join the convening and build a plan to take action on dementia risk reduction. The format of the Community Convenings both informs and motivates communities into action. Each step of the process is laid out on a specific timeline:
- Introductory Meeting (Virtual)
- Planning Meeting (Virtual) 2 Weeks following the Introductory Meeting
- Community Convening #1 4 Weeks following the Planning Meeting
- Community Convening #2 4 Weeks following the 1st Convening
- Debrief (Virtual) 1 Week following the 2nd Convening
- Follow Up Meeting (Virtual) 3-6 Month following the 2nd Convening
Eligibility: Local health departments in the United States who are not currently receiving dementia-specific federal funding. Applicants must be able to begin the process of hosting a Community Convening in May, June, or July of 2024.

Participation Timeframe: Beginning with the first planning meeting an approximate 12-week duration.
Support Provided:
- Administrative Assistance: BBS will take care of tasks such as creating invitations, developing agendas, tracking RSVPs, and documenting notes during the Community Convening meetings.
- Technical Assistance: Equipping the host with the tools and guidance needed to plan and execute their Community Convening effectively.
- Peer Learning Opportunities: To learn from the experiences of others in similar situations, including health departments that have participated in past Community Convenings
- Costs associated with the in-person events will be covered by the Alzheimer’s Association.
There is no direct financial support for this activity and sites will not be reimbursed for time spent or local travel.
