COVID-19 Reporting Procedure

With the continued and steady number of COVID-19 cases over the past two years, the UT Health Science Center is reminding employees that they are responsible for reporting positive cases and close contact exposure. Human Resources will no longer be individually reaching out to employee reports. All units and supervisors must direct employees to complete the COVID-19 Reporting Form and follow the appropriate CDC instructions for isolation or quarantine.

COVID-19 Reporting Procedure 

All employees must complete the COVID-19 Reporting Form for the following occurrences:

  • If you test positive for COVID-19
  • If you had close contact with someone who is COVID-19 positive
  • If you live in the same household as someone who is COVID-19 positive

In the interest of maintaining the health and safety of the campus, UTHSC has implemented the following safety procedure:

  • 5-day self-isolation for COVID-19 positive employees
  • 5-day quarantine following close contact for non-vaccinated employees
    • If you are up-to-date with your vaccinations and boosters, or have recovered from COVID-19 within the last three (3) months, no quarantine is required after a non-household close contact
    • However, you are expected to wear a mask, remain socially distant, and monitor for symptoms for a 10-day period
  • 10-day quarantine for employees who are residing with a COVID-19 positive individual
    • Employees with household exposures who voluntarily provide documentation of their up-to-date vaccination status are not required to quarantine.

  • ALL employees experiencing the above MUST request clearance by completing this form prior to returning to campus and submit it to University Health Services (UHS) for approval
    • For any questions, please contact UHS at eohs@uthsc.edu or 901.448.5630
    • UHS will provide authorization to return to campus and can advise employees on their unique health situations
    • If you test positive for COVID-19 and symptoms are not improving and/or a fever persists within the last twenty-four (24) hours, your isolation period may be extended to 10 days

Leave and Telecommuting

If approved by your supervisor and should your job duties allow, you may telecommute during your isolation or quarantine period. When unable to telecommute, accrued sick or annual leave must be used for work time missed. If leave balance is insufficient, leave without pay will be applied.


COVID-19 Vaccinations

UHS is no longer providing COVID-19 vaccinations. To locate COVID-19 vaccine near you, please visit https://www.vaccines.gov/.


All employees are encouraged to get tested (if they have had a close contact to someone testing positive for COVID-19, suspect exposure, or have symptoms) and vaccinated against COVID-19. For more information and further guidance:
