Individual EPLI Trainer Report

This form is intended for Current EPLI Trainers of START Content to report their training activity to START and their respective RCN.

Trainers are instructed to report all trainings they conducted using START content/materials.

This form should be submitted twice a year:
Mid-year deadline: January 31, 2025
End of year deadline: June 27, 2025

The data from this report will be shared with the RCN after each of these deadlines.

Please note: Trainers will be designated as Inactive if more than two years have passed without a report of a training. Inactive Trainers are required to reactivate according to the then-appropriate START reactivation process.
