The Office for Equal Opportunity (OEO) reviews concerns related to Sexual Misconduct, Sexual Harassment, Dating/Domestic Violence, Sexual Exploitation, and Stalking, as well as Discrimination, Harassment, and Retaliation as it relates to Protected Status* at Sac State for students, staff, and faculty. Once this form is submitted, our office will provide resources to students and employees, including where to receive confidential support, supportive measures, and information on how to file a complaint.** Please fill in all of the information requested below as completely as possible and attach additional pages to this form, if necessary.
We will review the reported concerns to determine if the concern falls within OEO's purview and jurisdiction. Submissions from this form are reviewed during normal business hours (8AM-5PM, Monday-Friday); we will be contacting the individual identified as the Complainant (i.e. person who experienced the concern) in the report within 3-5 business days to provide appropriate resources.
*Protected Statuses include: Age, Disability, Gender (or Sex, including pregnancy, childbirth, breastfeeding or any related medical conditions), Gender Identity, Gender Expression, Genetic Information, Marital Status, Medical Condition, Nationality, Race or Ethnicity, Religion, Sexual Orientation, and Veteran or Military Status
**Anonymous Complaints: OEO practices sensitivity and privacy rights regarding student and employees’ information that aligns with CSU policies. Per CSU’s Nondiscrimination Policy, the University may have limited capability to take further or full action if key information is missing, unavailable, or if the Complaining Party requests anonymity and/or does not wish to move forward with a complaint. OEO will make every good faith effort available to provide Parties with appropriate support, information, and reasonable action based on available information.

With limited exception, all University employees who know or have reason to know of allegations of Title IX, Discrimination, Harassment, and Retalorion conduct, are required to promptly inform the OEO. Employees must disclose all information including the names of the parties, even where the person has requested anonymity. OEO will determine whether such confidentiality is appropriate given the circumstances of each such incident. Completing this form satisfies employee reporting requirements.
