Case Management Form to Report Distressed Students


This form is used to notify Student Affairs of a student in distress or distressing student. Please note that submitted reports are generally reviewed during University business hours. If there is an immediate risk to health or safety, please call 911 or the Sacramento State Police Department at 916-278-6000.

The information you provide will allow the University to gather information and determine how best to assist a student in need. If you have questions about distressed student reporting, you may send an e-mail to cares@csus.edu
