Customize this Form to Collect your own Student Feedback

If you haven’t already seen it, I recommend Sarah’s post: Checking in: How to collect and respond to student feedback online which details the why and how of collecting student feedback.

This brief post is meant to share a student feedback template in google forms that you can adapt to utilize in your own course.

Finally, the most important step of collecting student feedback is reporting back to your students. Even explaining why you cannot change something provides greater transparency between you and your students.

Note: If you’ve used the Hub’s “Course Feedback” Survey in Canvas Commons, this survey is the same. When Canvas moves to new quizzes in the future, it will no longer support graded/ungraded surveys so the recommendation is to use a different survey tool, like google forms.
