Welcome to the University of California, Santa Barbara (UCSB) Incident Report Form.
Be Advised: This is NOT a Police Report. To make a report to UCPD, please fill out UCPD’s Report a Crime form at www.police.ucsb.edu/contact-us/report-crime.
For Emergencies and Crimes in Progress, please CALL 9-1-1 immediately.
To report incidents of Discrimination and/or Harassment, including but not limited to incidents of sexual harassment and/or violence, please submit a report to the Title IX Compliance & Discrimination and Harassment Prevention Office using the following link /uctitleix.i-sight.com/portal/Santa%20Barbara
Please note, the timeout limit for each report is 45 minutes. If the 45 minutes are exceeded, any information inputted prior to clicking the "Submit" button will not auto-save.
Clery / CSA Report: Select this form to report any criminal offense to the Clery Act Compliance Coordinator for inclusion in the Annual Security Report.
Academic Misconduct Report: Select this form to report an alleged violation of Academic Integrity. Form to be completed on behalf of or by the Instructor of Record.
Behavioral Misconduct (Non-Academic): Select this form to report an alleged violation of Non-Academic misconduct.
Student Organizational Misconduct: Select this form to report alleged behavioral misconduct matters involving UCSB student organizations.
