Community Collaboration for Health Equity Grant

2024 Application Period Is Now Open


Community Health Network is proud to announce the Community Collaboration for Health Equity Grant program. Community Health Network recently completed a Community Health Needs Assessment (CHNA) and corresponding Implementation Strategies. The CHNA identifies the significant health needs our community is facing and the Implementation Strategies defines how CHNw will work to address the identified needs over the next three years. In addition to our internal strategies, we are excited to partner alongside local, community-based organizations to improve health outcomes and health equity for our neighbors in Central Indiana. We're proud to extend this funding opportunity and look forward to partnering with you!

All applications must be submitted by 4:00 p.m. EST on May 1, 2024.

Significant Health Needs

Grant requests must address one or more of the following significant health needs identified in the CHNA:

  • Mental health and access to mental health services
  • Substance use disorders and overdose
  • Obesity, physical inactivity and chronic disease
  • Maternal, infant and child health
  • Social determinants of health
