Briar Cliff Athletics and CNOS Sports Medicine use the EMR software Sportsware to keep athlete and injury records. Student-athletes must submit their physical form, medical history form, insurance information and all required demographic fields on SportsWare. These records must be updated yearly and prior to competing in any team workout, practice or competition.
Physical Form
Print this form and give it to your medical professional so that he/she can complete and sign it. Make sure the BC student signs the bottom of the form. All student-athletes must have a current physical on file at Briar Cliff before participating in practice or competition. The physical form must be submitted on SportsWare.

Narcan Policy
Narcan/Naloxone may be administered by a university employee when there is reasonable belief that a person is suffering from an opioid overdose. Narcan/Naloxone nasal spray is supplied in a single 4mg dose of Naloxone hydrochloride in a 0.1ml intranasal spray.
