Thank you for registering your child to participate in Children’s Formation sponsored activities here at St. B’s for the program year! (Fall 2022-Spring 2023)

This form is for Sunday Formation Hour, special classes (such as Sacred Communion), and for Children’s Formation sponsored events where parents are not present in the room. Any other events will have a unique permission form based on the nature of the event (ex. Club 456 Events, Catechesis special events, Christmas Pageant, service projects).

Information provided on this form may be shared among pastoral staff and is held in confidence. If any information provided by you would help leaders serve your child better, Meredith Flynn would contact you to ask permission before sharing with leaders.
