The New Health Form is Live!

The new U.S. Health and Consent Form is now live and ready for summer 2026 (and beyond)! This new process replaces the need for areas to manage different health and consent forms for trips and events. There is now one streamlined form, managed in YL Connect, that guides participants through all required fields. This form will be required for summer 2026 and is available now for any trip over 72 hours.
For Nationally-Managed Summer Camp Trips and any trips longer than 72 hours:
As of February 2, 2026, if you have access to the camp trip in YL Connect, you can manage forms on the Health Forms tab. Use this new health form process for any camp trip over 72 hours, whether at a Young Life camp or not. See the Health and Consent Forms page on Staff Resources for detailed instructions on how to use the new form.

For Trips Less than 72 Hours or a High-Risk Event
Through May 2026, YL Connect will continue to use the 1717 health form process for trips that are either less than 72 hours or are high-risk events.
Starting in late May, all trips and high-risk events will use the new health form process. YL Connect and the campaign details will determine what information must be collected, and you can manage these forms on the Health Forms tab.
