COVID-19 Vaccines

  • We have Pfizer-BioNTech Comirnaty 2023-2024 and Moderna Spikevax 2023-2024 for persons 12+ years of age. Supplies are variable.
  • Health Insurance is required for vaccination to be covered or you may pay the vaccine expense. It is your responsibility to confirm that your insurance will cover the services. The combined charge for the vaccine expense and vaccination charge is $165.
  • We are not offering vaccines for children 2-11 years of age.
  • COMIRNATY and Spikevax are FDA-approved. Package inserts and Vaccine Information Sheets are available in many languages.
  • The COVID 2023-2024 vaccines are an Omicron XBB.1.5 sublineage
  • We are no longer providing printed vaccination records at your visit. All vaccinations are promptly documented in the Washington State Immunization Information System (WAIIS). You can download electronic and printable versions of records from WAIIS.
  • We are not permitted to enroll in the Bridge Access Program that offers no cost COVID vaccinations. These vaccines are only available through Public Health Clinics, Federally Supported Health Clinics, and a limited collection of pharmacies.

  • HEALTH INSURANCE IS NEEDED FOR VACCINATION , otherwise payment is due at the time of service.

Before you arrive

  • Please bring a photo-ID and your health insurance card with you. The vaccination may cost a copay depending on your health insurance. Teens can bring a school ID or be accompanied by a parent/guardian with photo-ID.
  • Please arrive 5-10 minutes early. Most people wait less than 5 minutes. It is still recommended that you wait 15-30 minutes after vaccination for medical post-vaccination monitoring.

FAQ

Can the person accompanying me get vaccinated also? No. We are scheduling all vaccinations on our website.

Will I get a vaccination card or have mine updated? No. Vaccination cards are no longer being used. All vaccinations will be promptly documented in the Washington Immunization Information System (WAIIS). You can download electronic and printable records from WAIIS. We recommend photographing the card(s) with your phone.

What side effects can I expect?

These are the side-effects seen in clinic trials of the Pfizer-BioNTech COVID-19 Vaccine. The full prescribing information is available online at cvdvaccine.com.

Many persons have used acetaminophen (Tylenol), ibuprofen (Advil), naproxen (Alleve) to limit post-vaccination symptoms.

We bill your insurance or the federal government for the vaccine and vaccination costs. The following CPT codes and charges may appear on your health plan Explanation of Benefits (EOB):

  • 90480 COVID Vaccine administration code
  • 91320 Vaccine Code for Comirnaty 2023-2024 12yrs+
  • 91322 Vaccine Code for Spikevax 2023-2024 12yrs+

YOU MAY HAVE A COPAY REQUIRED BASED ON YOUR HEALTH INSURANCE. The charges on your EOB for the CPT codes are two times the Medicare King County rate and are paid/adjusted based on contracted rates with health insurance plans. YOU MAY GET A BILL FROM US FOR YOUR COVID VACCINATION COPAY. YOU MAY ALSO GET A BILL FOR THE FULL $179 IF YOUR INSURANCE REFUSES COVERAGE FOR THE AMOUNT NEEDED TO COVER OUR COSTS OF PURCHASING THE VACCINE AND ADMINISTERING THE VACCINE.
