Hate/Bias Reporting Form


UWM is committed to creating and supporting a campus climate that is respectful and supportive of all who work, live, study or participate in activities or events in our campus community.

Members of the UWM community who have been subjected to, or who have witnessed, an incident of hate or bias are encouraged to submit a report using this form. Reports will be reviewed by UWM's Office of Equity/Diversity Services (EDS). After reviewing the report, a member of the EDS staff will contact the reporter to discuss the incident, to share information about campus and community support services, and to discuss options for addressing the concerns. If appropriate, EDS will share the information in the report with campus partners -- including the Dean of Students Office, University Housing, the Title IX Office, Facilities Services, and others -- to address the concerns.

If this is an EMERGENCY situation, CALL 911 or contact the UWM Police Department at 9-911 (from a campus phone) or (414) 229-9911 (from a cell phone or landline). Do not use this form to report events that present an immediate threat to health or safety.

Please know that, while we will make every effort to respect your privacy, in certain situations involving physical safety or potential criminal behavior, we may not be able to guarantee complete anonymity. In every case, reports will be handled discretely, and information about individuals or situations will only be shared with those who have a legitimate need to know the information.

 

While anonymous reports are permitted, UWM may be limited in its ability to investigate or respond to the incident if identifying information is not provided in the report. Therefore, anyone reporting an incident through this form is encouraged to provide a phone number and/or an e-mail address so that you can be contacted about the report.
