CLE Information

CLE Credits
Continuing legal education credit for the program has been approved in accordance with the requirements of the New York State CLE Board for a maximum of 13.0 nontransitional, professional practice credits.

Attorneys:
In order to receive CLE credit, the below affidavit and evaluation forms must be filled out in the links below within 30 days. After 30 days, the forms will no longer be available. Upon completion, the forms will automatically be emailed to the Fordham Law Events team. The forms are fillable PDFs.

** please note that you must be on time to all panels in order to get full CLE credit. The CLE codes will only be recited once, 10 minutes into the panel**

IMPORTANT: The CLE Affidavit and Evaluation forms can be filled out on the last day of the program if you are attending all three days. Please do NOT submit a form at the end of each day. If you are only attending one day, please fill out the form at the end of that day.

CLE AFFIDAVIT FORM

For the signature section of the Affidavit Form, on a touchscreen device, you can use your finger or a stylus to sign. On a desktop, you can use your cursor by left clicking on your mouse.
Signature is required for credit.

CLE EVALUATION FORM

Health and Safety

This year, to ensure safety for all participants and attendees, Fordham is requiring each attendee to abide by US CDC COVID vaccine recommendations. If you are under 50 years of age, two vaccination shots (or a one-time vaccination, such as J&J) and one booster shot are required. For those 50 and over, two vaccination shots (or a one-time vaccination, such as J&J) and two booster shots are required. If you reside outside of the US and do not meet these guidelines, Fordham will accept your current vaccination status as long as it abides by the vaccination requirements of your country of residence, in addition to proof of a COVID rapid test negative result within the prior 24 hours.

IMPORTANT MESSAGE for international guests, only: We will have free COVID rapid test kits available to be picked up TODAY, September 13th from 10 a.m. - 3 p.m. ONLY. Please, feel free to pick one up, to be taken off site, within that time frame at Fordham Law School's (150 West 62nd Street) main entrance. We will not have any available on the day of the program. COVID rapid tests cannot be taken on campus. Conference Speakers who are staying in a hotel booked by Fordham, will have a COVID test kit waiting at their respective hotels. To submit your results, please use this link. We require a picture of the results with your face in the image. You will still need to fill out the health form. If you have already filled out the forms, we have them already and there is no need to fill them out again.

To confirm your vaccination status with Fordham, please complete this health form. Additionally, we ask that you also bring a copy of your vaccination records to the event. Conference entry is contingent upon meeting these vaccination requirements. We look forward to seeing you this year!
