Vaden Entrance Health Requirements & Forms

All new students are required to complete the Entrance Health Requirements & Vaden Health Forms by August 1. Please review the requirements now to ensure you have time to contact your healthcare provider this summer, if needed.
To complete your Entrance Health Requirements:
1. Read Vaden Health Service's 2023-24 Student Health Matters booklet, your guide to caring for your health at Stanford, including information about the entrance health requirements and health insurance requirement.
2. Log in and complete your Health Forms online in the VadenPatient Portal. You'll need your Stanford ID (SUID) number - this is the 8-digit number found on your admission letter or on your Approaching Stanford Forms.
3. Upload your Immunization Record in VadenPatient.
Enrollment Hold: Students who do not complete the entrance health requirements by August 1 will have an Enrollment Hold placed on their account by Vaden Health Services, preventing you from enrolling in Autumn Quarter courses when enrollment opens for students: Aug 30 for transfers, during NSO for frosh.
Health Insurance Requirement: Student Health Matters also contains important information regarding the university health insurance requirement to maintain continuous health insurance, and the option to remain enrolled in or to waive Cardinal Care. Cardinal Care is the university-sponsored health insurance plan in which all students are automatically enrolled at the start of their entry quarter each year. Students with approved comparable insurance can choose to waive Cardinal Care.
  • International students planning to waive Cardinal Care should do so by August 15, otherwise you should acknowledge Cardinal Care by September 15 in Axess.
  • Domestic students will need to acknowledge or waive Cardinal Care by September 15 in Axess.
For questions about your individual situation and/or records, please contact Vaden staff by submitting a help ticket at stanford.service-now.com/student_services, select "Student Health." For general questions about the requirement, please post to the Vaden Forms Q&A Board in Canvas.

To Do in Axess

Student Information Update

All new students must complete the Student Information Update in Axess by August 1. You can now find the Student Information Update in Axess under "My Information". Please be sure to log in and complete the Update by August 1, or an enrollment hold will be placed on your account.

Axess Email Notification Settings

Did you know you can update your email notification settings in Axess to stop the daily emails about enrollment holds? In Axess, click on your Profile photo, then click on the Preferences tab where you can adjust your Email Notification Preferences. The New Student Advisement Enrollment Hold was placed for frosh on May 5, and for transfers on June 5. There is nothing you need to do at this time to resolve the holds and can ignore the email reminders.
  • For frosh: the hold will be automatically relased during NSO in September, at which time you will be able to enroll in your courses.
  • For transfers: the hold will be released by the Transfer Advisor later this summer after you've completed an advising conversation; transfers can then enroll in courses beginning August 30.
