"Using baseline data from the Nurse AMIE trial (R01CA254659; NCT04673019)—a technology-enabled supportive care intervention designed to improve symptoms and quality of life for patients with advanced cancer—this cross-sectional analysis examined differences in function, symptoms, and quality of life across the rural continuum. Patients living in the most rural settings (small town/rural RUCA codes) experienced worse physical function, greater symptom burden (including sleep disturbance and pain), and poorer quality of life compared with those in micropolitan areas. These findings demonstrate that rurality is not a monolith; meaningful heterogeneity exists within rural populations, with those in the most geographically isolated settings facing the greatest disadvantage. The results underscore a critical need for targeted physical activity and rehabilitation interventions to mitigate functional decline. Given the wide geographic dispersion and limited access to services, scalable technology-enabled approaches (e.g., telehealth-supported exercise) may be essential to extend reach and improve outcomes."
