Self-Certify for the Spring 2022 Semester
Dear Hornet,
As an incoming transfer student, you will need to self-certify your COVID-19 Vaccination status. Self-certification is required for ALL members of the Sac State community (students, administrators, staff, and faculty). Certifying your vaccination status is your ticket to accessing in-person classes or
on-campus events this spring.
You must self-certify by December 3, 2021, to be eligible to attend an in-person orientation session between January 5 and January 12, 2022.
Completing the self-certification form declares one of the following:
- You have received an approved COVID-19 vaccination, with the final dose administered at least 14 days before filing your certification
- You qualify for a medical exemption
- You qualify for a religious exemption
- You do not plan to access campus or any campus programs (including field placements or internships and athletics)
NOTE: Sacramento State estimates that as much as 70% of classes will be on campus this spring. If you declare number 4 (no plan to physically access the campus) and your plans change, you will need to revise your certification prior to accessing campus or a campus program.
