The Ask-Tell-Ask approach is a conversational tool that helps clinicians empower patients to participate in their healthcare decision-making process. Handouts and pocket cards can be downloaded from the Coalition's website.

Advance Care Planning Resources

Educational guides, webinars and more are now available from the HIGHway Project. The project, funded by the Patient-Centered Outcomes Research Institute (PCORI), aims to empower clinicians in dialysis centers to incorporate advance care planning into their workflows.
