Take the first step in your Partnership Promise.

To ensure you and your spouse (if he or she is covered on your insurance) get the most out of your Partnership PPO or Wellness HealthSavings CDHP you will need to complete the steps in your Partnership Promise. So cross step 1 off your list and complete your Well-Being Assessment online. It’s confidential, quick and easy:

  1. Sign in to your Well-Being Account to take the confidential Well-Being Assessment.
  2. Answer questions honestly about your physical, social and emotional health.
  3. Read the summary of your overall health and suggestions for improvement.

You may be called by Healthways to enroll in coaching to help you decrease your health risks and reach your well-being goals. If you are called you must participate. Calls are private and scheduled at times that work for you.

If your contact information changes at any time during the year, you and your covered spouse (if he or she is on your insurance) must update your information as soon as possible as part of your Partnership Promise.
