💉 DC law requires that students under the age of 26 provide proof of the immunizations listed on the AU Mandatory Immunization Form.
To submit proof of immunization, students must:
1-Log into the Student Health Portal.
2-Under the "My Forms" section, select "Immunization History" and complete the form with the dates of the vaccinations from their immunization record.
3-After submitting the form, select the link to upload a scanned version of their immunization history signed by their medical provider.
Records will be reviewed, and students will receive a secure message in their portal within approximately five business days.
Students who do not complete this requirement will be able to attend classes, however registration for future semesters will be denied until the requirement is met.
