We are pleased to share that the online submission process for One-Time Emergency Psychotropic Medication (STAT) through the Portal has been enhanced for greater clarity and ease of use.
What’s New?
The process for submitting for One-Time Emergency Medication(s) has improved. Now, when you indicate the submission is for a one-time emergency medication, you will see fewer required fields that relate only to one time emergency medication. You will no longer have to fill out all the fields for routine psychotropic medication requests.
What’s Staying the Same?
The process for submitting other types of Psychotropic Medication Consent Requests (such as New, Increase, Renewal, or New to DCFS, continuing medication) has not changed.

To request consent for psychotropic medication for youth in care, please complete the CFS 431-A: Psychotropic Medication form at:
https://guardianconsent.dcfs.illinois.gov/RequestsForConsent/
You will need a valid email address that can receive protected health information to complete requests online. After you submit the form, you’ll receive an email with a copy of your submission and a confirmation number. This email will include the health information you entered, so please make sure you use an email account that is secure and not easily accessible to the public or anyone not involved in the youth’s psychiatric care.
