| December 21st is the winter solstice—the longest night of the year. It is also Homeless Persons’ Memorial Day, and in many cities in the U.S. and around the world, people will stop to do what most people in their societies often don’t do—remember the men, women, and children who have died while experiencing homelessness or because of homelessness. Along with remembering on this day, they will hopefully reflect on the unnecessary tragedy of someone dying due to lacking a home and health care. This is significant because many in our society are unaware of the link between homelessness and poor health and premature mortality. Read more.

| Honor Homeless Persons’ Memorial Day (December 21) Homeless Persons’ Memorial Day (HPMD) brings communities together on or around December 21 to remember those who have died without stable housing, to reflect on the shocking inhumanity of homelessness, and to call for meaningful policy changes to ensure that no life is lived or lost in homelessness. Each HPMD event is unique to its community, but the commemorations often include name readings, candles, prayers, personal remembrances, marches, and moments of silence. Find an event in your community, learn how to hold a moment of silence for our lost friends, and share your thoughts on Twitter on how to achieve a world with #NoMoreHomelessDeath.

| Quick Guide Spotlights Strategies for Using PRAPARE and Other Tools to Address Homelessness Our new publication with the Corporation for Supportive Housing (CSH) provides practical tips for identifying social determinants of health (SDOH) faced by patients without homes using the Protocol for Responding to and Assessing Patients’ Assets, Risk, and Experience (PRAPARE) screening tool. SDOH are a critical piece of the health care continuum, and it is key to identify which social determinants patients experience to address their health care needs. To explore this issue, we recently partnered with CSH to conduct a learning collaborative identifying challenges and strategies from state and local health center perspectives. Emerging from these discussions, our quick guide provides recommendations for addressing these challenges using PRAPARE as well as other lessons learned.

| Applications Open for New Initiative on Diversity & Inclusion Racism is a principal driver of American homelessness, and organizations serving people without homes can be complicit in perpetuating it. It is vital that we embrace the racial diversification of the workplace and overcome the challenges of adopting anti-racist policies within our own institutions. In this context, the Council invites health centers and others serving people without homes to apply to take part in a Learning Collaborative on Diversity, Equity, and Inclusion. Meeting monthly starting in early 2019, this collective will address the most pressing topics facing homeless services organizations in the areas of racism and diversity. Members of the Health Care for the Homeless Clinicians’ Network are invited to join our new Clinicians’ Network Facebook group. This group is a forum for hands-on providers of any discipline to connect, ask questions, and share successes and challenges. Colleagues from across the United States will share their expertise and experience while engaging on issues related to health and homelessness in the group. As the 115th Congress concludes, stay up to date on the present and future state of health care policy in Mobilizer. Our latest issue examines policy proposals on the table before the lame duck session ends and where we expect health policy to go in 2019. It also issues a call to action to recognize gun violence as a public health problem, further single-payer legislation, and honor Homeless Persons’ Memorial Day. | New Opportunities Available for Free Online SOAR Training
The Substance Abuse and Mental Health Services Administration (SAMHSA) and Policy Research Associates are now offering two free online courses in SSI/SSDI Outreach, Access, and Recovery (SOAR). These online modules aim to educate case managers assisting adults and children who are experiencing or at risk of homelessness and have a serious mental illness, medical impairment, and/or a co-occurring substance use disorder to apply for disability income benefit programs from the Social Security Administration. Each course features seven classes culminating in the participants’ submission of a Practice Case Application Packet to the SAMHSA SOAR TN Center for individualized review and feedback.
