CURRENTLY ENROLLED: Students currently participating in the plan must re-enroll for the new policy year by December 1, 2023. Coverage will NOT be renewed unless a new enrollment form and payment is received by EBPA by the deadline.

NEW ENROLLMENT: To enroll for the first time in the Delta Dental PPO Plus Premier Plan, complete the Enrollment Form and Payment Form and submit to EBPA by December 1, 2023.

Please note: You are NOT automatically enrolled in or billed for the dental plan because the plan is a voluntary plan. Students who enroll in coverage are electing to be enrolled the entire calendar year.

Mail the Delta Dental enrollment and payment form to EBPA, 37 Industrial Drive, Suite E, Exeter, NH 03833.

Secure Portal Upload: Electronically transmitted through the EBPA Secure Document Submission Portal. Enter Tufts University under Employer Name or Group Number. Select Click here to submit claims, forms, or substantiation electronically. Then select COBRA and Retiree/Direct Billing to upload your document(s).
