Upcoming eForm Enhancements Launching October 27th |

As a reminder, several UCPath eForms are unavailable until
Monday, October 27, 2025 @ 8:00 am. Beginning Monday, October 27, UCPath Center will implement several enhancements to improve usability, accuracy, and efficiency across multiple eForms:

- Records and Data Request eForm
- Simplified to a single-page design to improve performance and reduce confusion when saving or submitting forms
- Accrual and Service Adjustment Request eForm

Added a Submission Reason drop-down menu to help ensure forms are submitted for a valid business reason
- Added new automated employee fields for Business Unit, Pay Group, and Eligibility Group
- Vacation/PTO Over Max and Employee AM Forms

Added new automated employee fields for Business Unit, Pay Group and Eligibility Group
- Updated header and balance details for clearer Vacation and PTO information
- Foreign Source Income Statement eForm
- The new eForm replaces the PDF form beginning October 27, 2025

Streamlines processing, reduces confusion, and improves data accuracy by integrating directly with UCPath
- Please remove any references or links to the PDF version of the Foreign Source Income Statement.
- Complete the new eForm by following this navigation path:
- Main Menu > Income & Taxes > Forms > Foreign Source Income Detail

The following enhancements aim to streamline processing for both employees and UCPath Center staff. For urgent matters during the scheduled downtime, submit an inquiry via Ask UCPath (Log into UCPath > click the yellow Ask UCPath button).
