Parent Signatures Needed by August 25 on Additional Back to School Forms for School Entry

Dear St. Michael's Families,
As a school community, we belong one to another, and we are stronger together than alone. It is
up to each of us to take measures on the School’s campus, at home, and in all aspects of our lives to keep us safe as a community. Our work together will control disease transmission, sustain our on-campus learning experience, and help protect those most vulnerable to COVID-19. We must form a strong partnership based on respect, responsibility, and relationship. COVID-19 mitigation guidance and mandates are evolving, and it may be necessary that our policies protocols and procedures evolve as well.
To that end, some additional forms have been added to PowerSchool/Magnus Health that require parent review and signature NO LATER THAN Wednesday, August 25.
These include:
  • An updated Parent Partnership Agreement -- required for school/campus entry
  • Proof of COVID Vaccination (for those that are eligible and received the vaccine)
  • Submittal of Birth Certificate (as part of updated school licensing requirements)
Additionally, all other Back to School forms that were due by July 15 MUST also be submitted no later than August 25, to include:
  • Vital Health Record
  • Consent to Treat Form
  • Athletic Participation Form
  • Annual School Physical Exam Form-- For Kindergarten, 7th grade, and all NEW students. (Must be completed by a physician — dated AFTER May 1, 2021 — and uploaded to MagnusHealth)
  • Action Plan Forms-- IF APPLICABLE for those with Asthma, Food Allergies, Diabetes, or Seizures (Must be completed each year with parent and physician signature. If your student will require their inhaler, epi-pen, or diabetic supplies at school, please include on the Action Plan.)
  • Field Trip Permission Form (found in PowerSchool e-forms)
  • Volunteer Personal Automobile Use Form (found in PowerSchool e-forms)

As always, the folks in the School office are here to help. If you have questions, please reach out at 804-272-3514. If you have trouble logging into PowerSchool, please contact Dave Grossman. We have also re-shared additional Back to School info below. We look forward to seeing all of our Dragons soon!

Sincerely,
Bob Gregg
Head of School

Remember Masks!

A friendly reminder that masks are required INDOORS on campus for all students, faculty/staff, and visitors, regardless of vaccination status.
Students should wear a mask to school daily and bring an extra mask in a plastic bag.
Universal masking is critical in our effort to deliver the full educational experience at St. Michael’s, meet the contact tracing parameters to the extent possible, and thereby maintain our in-person program. Thank you for your support!
