Delta Dental PPO Plus Premier Plan Open Enrollment for PhD Students

Dear PhD student,

Effective July 1, 2026, PhD students will have the option to enroll in individual dental coverage through the University at no cost under the Delta Dental PPO Plus Premier Plan.

Students who choose to enroll eligible dependents will be responsible for the dependent premium cost. The cost to add dependents coverage for the July 1, 2026 through December 31, 2026 coverage period is $338.34.

Please review the attached plan information for coverage details.

Coverage for this enrollment period will run from July 1, 2026 through December 31, 2026.

The deadline to enroll is June 15, 2026.

Enrollment forms submitted after June 15, 2026 will not be accepted.

Please note:

Enrollment in the dental plan is optional.
The dental plan is separate from the health and vision insurance plan.
You are not automatically enrolled.
If you choose to enroll, you will remain enrolled until December 31, 2026.
Information about future enrollment periods and 2027 coverage will be shared at a later date.

To enroll in the Delta Dental PPO Plus Premier Plan:

Complete and upload the Enrollment Form using EBPA’s Secure Document Submission Portal by June 15, 2026.
Complete fields 4 through 12 on the Enrollment Form, including:

Social Security Number.
Contact information.
Incomplete forms may delay processing or prevent enrollment.

If you want to enroll eligible dependents:

Complete sections 14 through 18 of the Enrollment Form
Complete and upload ACH Payment Formalong with the Enrollment form
Cost of adding dependents(s) for this coverage period is $338.34

EBPA Secure Portal Instructions:

Go to link EBPA Secure Portaland complete all required fields
Enter "Tufts Voluntary Dental" as the employer name.
Select "Election Form" for the file type.
Upload your completed forms.
Click Submit.

This is a PPO plan and can be used outside of Massachusetts.
