Please don't sign/witness blank beneficiary forms
We all like to help others, but when it comes to being a witness to a beneficiary form, it is important to realize you are attesting by your signature that the form was completed in your personal presence.
If you do choose to be a witness for your employees, make sure they complete the form in your presence.
Submitting beneficiary designation forms
Even though it is the member’s responsibility to complete and submit beneficiary designation forms to TRSL, some employers may choose to collect completed beneficiary forms and submit on behalf of their employees. 

Please mail completed forms to TRSL immediately! Waiting to send TRSL completed beneficiary forms until you’ve collected a large influx will delay TRSL’s receipt of the completed forms. In the event of a TRSL-covered employee’s death, only the most recent beneficiary form on file at TRSL will be valid. Completed forms on file with the employer that have not yet been received by TRSL cannot be used. 
On a regular basis, it's a good idea to encourage your employees to check their beneficiaries through Member Access and submit a Beneficiary Designation for Non-Retired Members (Form 3) when necessary.
