REMINDER... Disability retirement requires the following certifications in addition to that of service credit and sick leave:
Disability Report by Supervisor (Form 12A), which must be completed by the employee’s supervisor. If the supervisor is unavailable, you may be required to complete it.
Workers' Compensation Certification form. This form (see below) will be sent to the employer requesting information if the disability applicant indicates workers' compensation benefits were received during employment on Section 2 of the Application for Disability Retirement (Form 12).
