MISHC Updates:

• 2022 Q4 MISHC Reports are available on the password protected MISHC.org website
  • Fixes in denominators for 1-year rolling 4-quarter follow-up report
  • 2022 Q4 discharge level, 30-day follow up, 1-year follow-up and M&M reports available now
  • 3-year R4Q reports to follow
  • Risk-adjusted reports to follow

• MISHC Quality Improvement Project Update
  • QI Project Plan and Summary Form can be found on MISHC.org under "Coordinator Resources" and "General Resources."
  • Sheryl will be asking you to share your projects at the July Coordinator Virtual Meeting
  • Reach out to Sheryl if you need help in determining a quality project

• 2025 VBR Goal Survey
  • VBR Goals will be discussed at the June 28th, 2023 MISHC Collaborative virtual meeting
  • Survey will be sent to physicians and coordinators
  • Reach out to Sheryl if you need help finding your site’s performance in the proposed goals
  • Be sure to give your physicians your site’s performance for the following:
    • Cardiac Rehab referral prior to discharge
    • KCCQ Completion at 1 year
    • Shared Decision Making documented
    • AV Mean Gradient documented at 1 year

Save the Date:

• The MISHC Collaborative Webinar will be held on June 28th, 2023
• The Michigan Cardiac Rehab Network (MiCR) Stakeholder Meeting will be held in-person on October 27th, 2023 from 10 am - 3 pm. Location TBD.  We encourage you and your team to attend!
• The MISHC Coordinator/Abstractor Meeting will be held in-person on November 10th, 2023.  Location TBD. 
• The MISHC Collaborative Meeting will be held at the VistaTech Center at Schoolcraft College on November 11th, 2023 from 9 am - 12pm. This meeting is in collaboration with MSTCVS.
