    COACHES

    • It is your responsibility to know which of your Athletes submit the Medical Referral and Special Release Forms
    • You must read them and know what, if any, restrictions are listed for those Athletes
    • You are responsible to ensure that you follow the instructions from the doctor concerning what restrictions or limitations are allowed for competition

      NEW 3-Year Medical / Registration / Release Forms

        Spinal Cord Compression (SCC) & ATLANTO-AXIAL INSTABILITIY (AAI)
        • The Doctor will determine if there is NO evidence of symptoms for SCC or AAI or  Athlete has neurological symptoms that could be associated with SCC or AAI
        • If there are symptoms found by the Doctor – the athlete must receive an additional neurological evaluation to be eligible to participate in Special Olympics
        • If the Doctor checks the box on the Physical Exam – page 3 - indicating the athlete has symptoms and must receive an additional neurological evaluation – the Athlete must also submit the Medical Referral Form and the Atlanto-Axial Instability (AAI) Special Release Form

          Medical Referral Form
          • Will be used only if the Doctor does not clear the athlete and refers the Athlete for further evaluation 
            • If this form is required – it must be scanned and submitted with the Athletes medical forms

            Atlanto-Axial Instability (AAI) Special Release Form
            • This release form is only completed if symptoms of SCC or AAI are present
              • If this form is required – it must be scanned and submitted with the Athletes medical forms

                NEW Volunteer Policy & FAQs
                • A new Volunteer Policy, FAQs & Code of Conduct are now available here. All Class A volunteers age 14+ must complete all Class A requirements
                  • Volunteer Application – Protective Behaviors – Background Check  
                • New process for Protective Behaviors – SOI now requires all volunteers to create a profile in the Special Olympics Learning Portal
