This is a reminder that the deadline to submit for re-enrollment in the optional dental plan is 12/1/24. Please review our previous message for details.

Forms must be uploaded to the EBPA’s secure portal or postmarked by 12/1/24. No exceptions will be made for forms uploaded or postmarked after 12/1.
