Our goal at Laguna Honda Hospital (LHH) is to provide a welcoming, caring and healing environment for our residents. To foster this culture, we strive to be communicative and transparent with our LHH residents and their loved ones, our staff, and the larger community.
I am writing to inform you that Laguna Honda is undertaking a review after a resident on the North 1 (N1) neighborhood was suspected to have had a non-prescribed medication in their system. The resident recently passed away; however, there is no suspected connection between the non-prescribed medication and the resident’s passing.
Laguna Honda is deeply committed to creating a safe environment for all our residents. For this reason and per our policies, we are working closely with the San Francisco Health Network as well as our partner agencies at both the City and State level to conduct a review of the incident.
In the coming weeks, residents and staff will be interviewed, and safety searches of residents will take place throughout the hospital. These searches on North 1 have already begun. Any unexpected items will be addressed on a case-by-case basis.
I am confident this review will result in a stronger LHH community. While no additional details regarding the review can be shared at this time, we will inform residents and resident families of the key findings upon completion. Please know that the review will be extensive and exhaustive.
Laguna Honda takes very seriously the presence of any unprescribed medications or controlled substances on our campus. Please contact your resident care team if have any information that you think would be relevant to our review.
Thank you for your commitment to our community’s safety and well-being, especially during these challenging times.
Sincerely,
Michael T. Phillips, MHA, FACHE
