Our records reflect that you are not compliant with the
UC SARS-CoV-2 Vaccine Program (COVID-19 Vaccine Mandate) that went into effect July 15, 2021. The COVID-19 mandate required you be fully vaccinated and provide proof, or submit an exemption request, to UC San Diego by September 6, 2021. Therefore, you
may be in violation of the following University Standards of conduct:
SCP 4(e) (Violating health and safety requirements) - Violating health and safety requirements issued by the University and/or local, state, or federal governments in response to public health emergencies (e.g., COVID-19).
PACAOS 102.16 (Failure to Comply/Obstruction) - Failure to identify oneself to, or comply with the directions of, a university official or other public official acting in the performance of his or her duties while on university property or at official University functions; or resisting or obstructing such University or other public officials in the performance of or the attempt to perform their duties.
OPPORTUNITY TO PROVIDE PROOF OF COMPLIANCE/CORRECT RECORD
You have until September 29, 2021 to come into compliance, or correct your compliance record, by taking one of the following actions:
A. You are planning to be fully remote for fall quarter and not coming to campus for any reason between now and December 31, 2021:

4) Upon review and approval, you will receive a copy of your completed form via email
OR
B. You are vaccinated:
1) Submit evidence of your vaccination information into your MyStudentChart
2) If you are unable to upload evidence or see a discrepancy, send Student Health Services a message through the “Ask a Nurse” function in MyStudentChart.
As a reminder, students not in compliance with the COVID-19 Vaccine Mandate should not be physically present on campus or at university-supported activities. Failure to adhere to this or follow other COVID-19 policies and requirements applying to students, including deadlines when applicable, may violate the University Standards of Conduct and will be referred to the Office of Student Conduct to be addressed.
Students not in compliance because they submitted their exemption request late are permitted on campus property and at university-sponsored events on the condition they observe non-pharmaceutical interventions (NPIs) until they receive a decision. NPI examples include staying home, especially when a person is sick or when a member of the person’s family or household is sick; quarantining when an unvaccinated person has been exposed to someone else with the illness; avoiding large gatherings; physical/social distancing; wearing personal protective equipment or face coverings; frequent handwashing and cleaning; and asymptomatic (surveillance) and symptomatic testing.
NONCOMPLIANCE/ PROGRESSIVE ACTION
If you fail to provide proof of your compliance with the COVID-19 Mandate, or correct your compliance record, by the deadline, the following actions may be taken:
- Student Conduct Referral. (September 30th, 2021). Referral to the Office of Student Conduct to be addressed through the student conduct process and progressive sanctions (e.g., probation, suspension, exclusion from campus and/or University-supported activities, or other sanctions).
- Housing Contract Actions. Students living in university owned, operated or leased campus housing that fail to comply with University policies may also have their housing contract terminated.
- Student Account Hold. Student Health Services will place a hold preventing class add/drop, registration, auto-enrollment off waitlists, and other university business. Their student account hold will remain in place until they become compliant. Only after a hold is lifted may students enroll in classes or auto-enroll off class waitlists. Therefore, it is imperative you become compliant as soon as possible, to avoid impacts to your Winter Quarter enrollment and expected time to graduate.
- Disenrollment. Students remaining out of compliance with the COVID-19 Vaccine Mandate will be dis-enrolled in advance of the Winter 2022 quarter.
