Advance Care Planning Implementation Tools

- Use the HIGHway Project's guides to help your dialysis center teams incorporate advance care planning into their workflows.

-The HIGHway Project's website also has case studies, conversations with patients, webinars and more. 

Exploring the evidence base for Active Medical Care without Dialysis (AMCWD)

- In the ExPAND Project (led by co-principal investigators and Coalition Executive Committee members Dale Lupu and Alvin "Woody" Moss), participating sites will be implementing kidney supportive care programs for those patients who choose care pathways outside of dialysis.

-The Coalition has made available an engaging evidence base that helps clinicians understand the key points regarding AMCWD in older adults.
