The Graduate School Supervisor Evaluation Form comprises of a GA self-reflection, mentorship support offered and open-ended sections. The reflection is completed by both the supervisor and the GA after which they discuss, sharing recommendations for a way forward. In the mentorship section the supervisor is meant to rate the support they have provided to the GA to improve capacity etc., while the open-ended section informs the department on the improvements required and general feel of the supervisor about the GA.
