CURRENT/EMERGING ISSUES & STRATEGIES FOR SUCCESS

1. Increasing numbers of positive cases requires site-wide testing that often produces more positive cases, which then requires additional site-wide testing. This is exhausting for staff.
2. Ongoing uncertainty surrounding the closing of alternate care sites makes planning staff support difficult.
3. Impending winter weather increases the urgency for safe shelter and housing resources.
4. Surging numbers of street homeless individuals and increased street sweeps diverts advocacy and outreach services away from vaccine efforts.
5. Awaiting CDC guidance on the third vaccine dose and attempting to coordinate with flu vaccine campaigns.
6. Determining whether to offer the COVID-19 vaccine and flu shots simultaneously or separately.
7. Talking with patients who continue to decline vaccines due to misinformation from social media is exhausting, frustrating, and disheartening to staff who work hard to build trust (and trustworthiness) with their clients.
8. Stepping back from community COVID-19 response efforts because of the demands of chronic care management and not having enough staff to do both.
9. Increasing demand for testing, especially for youth who have had an exposure at school and need a negative test before being able to return.
10. Anticipating increases in newly homeless individuals as the federal eviction moratorium ends. 
    

1. Contact local and state health departments regarding available funds for testing and advocate they be used to serve people experiencing homelessness.
2. Use medical respite beds to provide monoclonal antibody treatment for individuals eligible for post-exposure prophylaxis.
3. Partner with hospitals for training on and access to monoclonal antibody treatment.
4. Plan to combine flu shots with COVID-19 vaccines to engage people who are unvaccinated.
5. Offer town hall discussions and vaccine clinics with testing after staff or residents test positive within a shelter.
6. Continue symptom screening to catch cases that may have gone undetected as patients return to clinic for services (not just COVID).
7. Bolster access to Pfizer vaccines as youth return to school and people aged 12-15 are seeking vaccines.
8. Advocate for local and state extensions of eviction moratorium to allow clients time to access financial resources and remain housed.
9. Create additional flexibilities in housing program rules regarding substance use to allow greater opportunities to stay housed and prevent homelessness.
10. Begin winter shelter/Code Blue preparation in anticipation of the cold weather. This should include plans to extend alternate care sites beyond the current funding period that ends December 31.

• Johnson and Johnson/Janssen announced that providing a booster dose after their initial 1-dose COVID vaccine produces a rapid and robust increase in spike-binding antibodies.
