HCH Convalescent Care Program in Baltimore, Maryland
Council to Receive $2.6 Million for
Medical Respite Study
The National HCH Council is one of 39 Health Care Innovation Award recipients announced by the U.S. Department of Health and Human Services. The $2.6 million award is administered by the Center for Medicare and Medicaid Innovation (CMMI) and will be used to demonstrate improved health outcomes and reduced spending when homeless patients have access to medical respite care following a hospital stay. Community Health Worker Spotlight
The National HCH Council is pleased to highlight the staff of Heartland Health Outreach as one of the sites participating in the Council’s Health Care Innovation project, Community Health Workers and HCH: A Partnership to Promote Primary Care Since Rodney Dawkins took on the role of CHW at Heartland Health Outreach, he has worked tirelessly to engage and connect clients with much needed services. An essential mission of HCH is to empower those we serve. Exercising the right to vote is a fundamental part of engaging in a democracy, even if one lacks a stable address. Got Medicaid?
The Council and its partners are putting a high priority on getting all states to “yes” on Medicaid, but we need your help in the effort! The Got Medicaid? campaign was launched at this year’s National HCH Conference and has continued to develop, with a new web page with resources and regular posts on the Council’s Facebook and Twitter pages. Grant Awarded for Women and Alcoholism Study
Members of the Council’s Research Team are part of a recently awarded grant for the study, Substance Abuse, Mental Health, and Health in Homeless Women in Primary Care. The Exploratory/Development grant from the National Institute on Alcohol Abuse and Alcoholism was awarded to the University of Massachusetts Medical School. Danger! Health Center Funding Cliff
Health Centers face a precipitous drop in federal funding next year without Congressional action due to the expiration of Affordable Care Act funds. Letters supporting the Health Center program and the need to address this "funding cliff" are an essential step towards passing legislation to keep the program funded at current levels. Contact your House and Senate members and ask that they sign on today!

On the "Careers"  page, grantees and others may post job descriptions and application details. View open positions at HCH programs across the country and post your own job announcements.
