High Quality Data for High Quality Population Health Management Programs

Today’s population health professionals have access to more data than ever before to help guide effective, engaging healthy lifestyle interventions. But where do you start, and how can you ensure you have quality data? As technology advances, gathering personalized data is the key to more targeted intervention programs that can increase engagement and improve health outcomes. When it comes to collecting data, garbage data in, garbage data out.

This webinar will explore the importance of quality metrics for self-reported data collection using an health risk assessment, and the ways the data can be used to inform a more effective population health program.

Join in the conversation on September 15th as this panel of industry practitioners discuss what makes for quality population health data – and how to best put it to use in population health programs. Attendees will hear tips to ensure you are collecting and using quality self-reported health and lifestyle data, standardization efforts across quality measures — and how they promote a comprehensive, effective population health strategy, firsthand accounts of quality improvement strategies, and how your technology vendors can be your best partners when it comes to creating quality programs, and an overview of quality measures and program recommendations from health plan accreditation body NCQA

Presented By:
- Chris McReynolds - CEO at Wellsource
- Natalie Mueller - Senior Product Development Manager at NCQA
- Sarah Emanuel - Manager, Wellness Services at Blue Cross and Blue Shield of Nebraska
