While some serve on the front lines and others work from home, all pop health professionals are collectively working to best support the health, wellbeing, and safety of our communities. Population Health Management is the official journal for the Population Health Alliance with David Nash, MD MBA serving as Editor in Chief. PHA members receive a full online subscription as part of membership. A recent journal study found that those over 65 who self-reported as socially isolated were more likely to have a future hospital admission or emergency room visit.
