Submit Medication Forms and Updated Vaccination Reports |
Please submit all outstanding medication forms and updated vaccination reports by accessing the Health Forms & Policies Tab under Parent Resources on the ULS website.
A message from Brooke Piper, School Nurse:
I began at ULS last October as substitute in the nurse's office and am now here on a full time basis! I am coming from Baton Rouge General where I have worked in both ICU and PACU for the last 6 years. I have 2 young children at home, so I am very familiar with all of the runny noses and coughs that will come to me this year. My goal this year is to streamline the services provided by the nurse's office and help you to navigate all of the healthcare paperwork that allows us to provide those services. Take a peek at our new Health Forms & Policies tab on our Parent Resources section of the U High website to see some of the changes we've made so far! I look forward to meeting your children (hopefully not too often!) and am here as a resource to reach out to for any health related needs your child may have while they are at school.
