Back to School Health Forms DUE AUGUST 1
Dear St. Michael's Families:
For the 2024-25 school year, St. Michael's will continue to capture all student medical information into a secure online database. Parents must enter and update the required information for each child this summer through the Magnus Health online portal (instructions below) accessed from your PowerSchool Parent account.
The deadline for the completion of forms is August 1.
Please complete each of the following sections on your Magnus Health TO DO list with the corresponding Action button for each section:
- Conditional Questions
- Vital Health Record
- Consent to Treat Form
- Athletic Participation Form
- School Entrance Health Form (For Kindergarten and all NEW students. Must be completed by a physician — dated AFTER May 1, 2024 — and uploaded to MagnusHealth)
- Updated Immunization Record for all 7th grade students per Commonwealth of Virginia Immunization Requirements
- Action Plan Forms-- IF APPLICABLE for those with Asthma, Food Allergies, Diabetes, or Seizures (Must be completed EACH YEAR with parent and physician signature. If your student(s) will require their inhaler, epi-pen, or diabetic supplies at school, please include on the Action Plan.)

- Volunteer Personal Automobile Use Form
- St. Michael's Acceptable Use Policy
- Field Trip Permission Form
- 2024-2025 Concussion Information and Policy Acknowledgement
