Please join us April 5 for a virtual half-day symposium on treating Hepatitis C in primary care settings among people experiencing homelessness. We'll cover a wide range of topics specific to treating Hepatitis C in this population, including substance use, harm reduction, and complex medical needs.
Immediately applicable clinical information will be provided on conducting pre-treatment assessments, common barriers to treatment, addressing complex clinical scenarios, models of care delivery, integration of Hepatitis C treatment into primary care and outreach services, and lessons learned from provider and patient experiences. Breakout sessions will address getting started with hepatitis treatment, strategies for successful treatment completion, treating Hepatitis C in people with advanced liver disease, and addressing common insurance and policy barriers. Along with the Symposium, the National Health Care for the Homeless Council and Swope Health will be releasing a Hepatitis C Treatment Guide to support primary care teams.

Community Conversation: Medicaid Redetermination

The nearly 92 million Americans enrolled in Medicaid must once again complete annual renewals to demonstrate their eligibility. For some, this will happen automatically while for most it will be a confusing, frustrating, and arduous process. It is estimated that almost 7 million Medicaid enrollees will lose their coverage despite still being eligible. Every state must begin this process no later than April 1, 2023. Join our community conversation on March 23 at 4 p.m. CDT to discuss how you can help Medicaid beneficiaries in your community complete the redetermination process and stay enrolled in Medicaid.

Over the past three years, there has been an increasing awareness of the inequities that undergird our systems. Ethical dilemmas with a lack of resources have exacerbated the moral injury Health Care for the Homeless providers face daily. The intersectionality of the COVID-19 pandemic increased the impact on marginalized communities and the provider(s) who care for them. In a “post-COVID” world, how do we navigate the path ahead? Join us March 29 at 2 p.m. CDT for a discussion with others as we learn to navigate ethics, equity, and moral injury together.

| Community-Centered Approaches to Health Care: Introduction to the Community-Centered Health Home Model (First of 2 sessions)

Health Care for the Homeless providers serve a population with complex health and social needs. One of the main challenges to improving health for people experiencing homelessness is the ability to address the social determinants of health including access to housing, food, transportation, employment, and other resources. Building innovative, cross-sector community partnerships is one way to accomplish this goal.
Developed by the Prevention Institute, the Community Centered Health Home (CCHH) model builds on the concept of PCMH while integrating a whole person, SDOH framework grounded in equity and justice. A CCHH model not only acknowledges that factors outside the healthcare system affect patient health outcomes, but actively participates in improving them. This is a two-part webinar series in partnership with the Prevention Institute, with part one exploring the CCHH model and part two diving into operationalizing a community-centered approach. All health centers collect and report UDS data, but not all are utilizing race and ethnicity data aimed at improving equitable outcomes. We will discuss how the UDS is limited and often not the best reflection of the high-quality care provided at HCH clinics. Additionally, data extraction is common from marginalized populations. How do we collect and share data that is meaningful to the community we serve?
