The National Institute for Medical Respite Care (NIMRC), a special initiative of the National Health Care for the Homeless Council launched in July 2020, is extending a call for qualifications to pre-qualify individuals, firms, nonprofit organizations, and academic institutions to serve as Affiliate Consultants. A first-of-its-kind national institute, NIMRC advances best practices, delivers expert consulting services, and disseminates state-of-the-field knowledge in medical respite care.
Along with internal staff of NIMRC and the Council, Affiliate Consultants will respond to the rapidly expanding demand for expertise to address the needs of existing and start-up medical respite care programs. From the pool of qualified Affiliate Consultants, NIMRC will contract for consulting services based on the scope of work, availability, geography, and best fit. Applications are encouraged from people with lived expertise in homelessness, as well as Black, Indigenous, and people of color.

| New Council Learning Collaborative Focuses on Community Centered Health Homes

Health Care for the Homeless providers serve a population with complex health and social needs. The Community Centered Health Home (CCHH) model builds on the concept of Patient Centered Medical Home while integrating a whole person, Social Determinants of Health framework.
The National Health Care for the Homeless Council has partnered with the Prevention Institute to provide trainings and peer learning sessions to assist HCH health centers in the adoption of the CCHH Model and support the participating health centers as they build community partnerships to accomplish this goal. This is a three-year project to ensure that participants have the support needed as they implement a new model. Participants will use the framework of a Change Map as an action planning tool and will use the Prevention Institute’s CCHH materials for assessment and to inform their action plans.

| Virtual Care Innovation Network: Redesigning Care

The Council is partnering with the Center for Care Innovations (CCI) and Kaiser Permanente as part of the Virtual Care Innovation Network, which aims to bring together safety net organizations across nine states to redesign care so that virtual care models continue after the COVID-19 pandemic abates and beyond.
The program will enable participants to learn from peers and experts, test new approaches, accelerate the work they have already started, and develop ways to sustain virtual care as an essential component for how care is delivered into the future.

Your Feedback Needed on Health and Housing

Are you looking to strengthen your links to housing resources in your community? Are health outcomes for your patients impacted by gaps in community resources? The Corporation for Supportive Housing (CSH) is hosting a series of focus groups to help design user-friendly and accessible tools for a Health and Housing Partnership Guide. The goal of these focus groups is to gather feedback and input from health centers and PCAs to help determine how they can promote understanding of the content for easy and effective learning on health and housing partnerships.
There will be five focus group sessions held in November. CSH welcomes a mix of perspectives including clinical, enabling services, outreach and administration. E-gift cards are available for your time and participation.
