Important Changes to Provider Reporting of Diseases and Conditions in LA County

The Los Angeles County Department of Public Health (LAC DPH) has transitioned most non-urgent Title 17 provider case reporting to the California Reportable Disease Information Exchange (CalREDIE) Provider Portal.
- Providers and individuals who submit reports but do not have a CalREDIE account specific to LAC DPH are encouraged to register now.

LAC DPH has developed a new reportable disease webpage with a search tool that helps users determine how to report required diseases and conditions, including non communicable cases and those reportable to external agencies.

LAC DPH is modernizing how providers submit mandatory case reports to improve timeliness, completeness, and ease of use by transitioning the reporting of most non‑urgent diseases and conditions to the CalREDIE Provider Portal.
CalREDIE Provider Portal
CalREDIE is a secure online system to replace mail, email, and fax-based case reporting. While traditional submission methods continue to be accepted, use of the CalREDIE Provider Portal is strongly encouraged. Providers who do not already have an LAC DPH–specific CalREDIE account are urged to register now. Visit the Provider Portal webpage to learn more and register for an account.
Note that this transition does not apply to the following:

- Diseases that require immediate reporting, including outbreaks and occurrences of any unusual diseases
- HIV reporting
- Hospital discharge approvals for tuberculosis
- Animal bites to humans

New Reporting Webpage and List
|
LAC DPH has launched a new Reportable Diseases & Conditions webpage. The page features a search tool that allows clinicians to quickly locate reporting requirements, resources, and procedures for any reportable disease or condition. It includes diseases that are reportable to external agencies, and communicable and non‑communicable conditions, such as silicosis, pesticide-related illnesses, disorders involving lapses of consciousness, Parkinson’s disease, other neurodegenerative diseases, cancer, and benign or borderline CNS tumors.
A new downloadable summary list of reportable diseases and conditions is also available on the webpage.

Actions Requested of Providers
|
- Request access to an LAC DPH-specific CalREDIE account (even if you already have a Provider Portal account for another jurisdiction) by completing the CalREDIE Provider Portal Account Authorization Form.*

Report non-urgent Title 17, Section 2500, California Code of Regulations
reportable diseases and conditions via CalREDIE as soon as your account is activated. -
Report cases in patients who reside in Long Beach, Pasadena, or any jurisdiction outside Los Angeles County, to the appropriate local health department (see resources below).

*Clinicians may delegate appropriate staff to submit reports on behalf of their facility.

Complete and accurate reporting provides important surveillance data that helps Public Health identify disease trends, detect clusters and outbreaks, and guide timely prevention and control measures. Delayed or incomplete reporting can hinder public health response and may contribute to preventable secondary transmission and outbreaks.
Reporting is required by law. Failure to comply with applicable reporting requirements can result in significant legal and professional consequences. The About Mandatory Reporting webpage provides information on reporting requirements and relevant laws, including a list of 8 conditions that must be reported to LAC DPH per local Health Officer Order, in addition to those required by federal and state regulations.
