Proof Form Process Coming in August
Proof forms are a way for your practice to receive Provider Incentive Program (PIP) payments for incentivized services rendered in 2024, where a Medicaid claim may not have been filed. An example of this could be a patient seen for a well-visit who has Medicaid as a secondary insurance. This process will begin at the end of August and will be complete by the end of October, so PIP payments will be on the quarterly check in November. Your provider relations team will contact practice leaders once this process is open, and share what documentation is considered an "acceptable proof." For more information, contact provider relations at PFKProRelations@childrensdayton.org.

For Primary Care Providers: Asthma Inhaler Patient Assistance Programs

This resource is not intended for patients with active coverage through Ohio Medicaid plans. For Ohio Medicaid inhaler coverage, please review the Partners For Kids’ UPDL.
