Dear Student,
Please read the Delta Dental insurance cover letter regarding the optional Delta Dental PPO Plus Premier Plan that is being offered for the next calendar year. The new policy year will run January 1 to December 31, 2021. Also included are the rates, coverage details, and instructions to enroll in the plan. The deadline to enroll or re-enroll is December 1, 2020. Students who enroll in coverage are electing to be enrolled the entire calendar year.
IMPORTANT ENROLLMENT INFORMATION:
- CURRENTLY ENROLLED: Students currently participating in the plan must re-enroll for the new policy year by December 1, 2020. Coverage will NOT be renewed unless a new enrollment form and payment is received by EBPA by the deadline.
- NEW ENROLLMENT: To enroll for the first time in The Delta Dental PPO Plus Premier Plan, complete the enrollment form and payment form and submit to EBPA by December 1, 2020.
Please note: You are NOT automatically enrolled or billed for the dental plan since the plan is a voluntary plan.
Mail required documentation and payment information to EBPA, 37 Industrial Drive, Suite E, Exeter, NH 03833. When selecting a dentist, please make sure to select in the Delta Dental PPO network.
