Mandatory Pre-Entrance Health and Health Insurance Requirements |

Hello incoming graduate students!
To maintain the health of our Jumbo Herd, we have several pre-entrance health requirements that you need to complete. Pay attention to these deadlines, as missing them could result in a hold on your registration or additional charges that cannot be refunded.

You can find details and instructions on the requirements listed below on the Health and Wellness Requirements for New Students website.

- Pre-entrance Health History Form: Available now on the Patient Portal. Must be completed by July 1, 2024.

Tuberculosis (TB) Risk Assessment Questionnaire: Available now on the Patient Portal. Must be completed by July 1, 2024.

Required Immunizations (Vaccinations): Provide documentation now on the Patient Portal. Must be completed by July 1, 2024.

- If you do not complete the Health History Form,TB Risk Questionnaire, and provide documentation of immunizations by July 1, this will result in a hold being placed on your Student Information System (SIS) account. You will be unable to register for classes until these requirements are complete and reviewed.

If you have any questions or cannot complete the requirements due to extenuating circumstances, please send a message to the Immunization Reviewer through the Patient Portal.

Health Insurance (either waive student health insurance with your existing policy or be automatically enrolled)
As a first-year graduate student, you will be enrolled in and charged for the university’s health insurance plan. If you have comparable insurance coverage through another plan, you can apply for a waiver of the university’s plan between now and July 31 in the Student Information System (SIS).

If you already have comparable health insurance and you do not apply for a waiver by July 31, you will be charged for the student health insurance for this year. Disenrollment or refunds are not available after July 31. Voluntary disenrollment is only available upon entering the armed forces.

- (Optional) Tuition Insurance
Available later this summer on A.W.G. Dewar’s website. Enrollment must be before September 3, 2024.
