Quality Improvement Update
Our Quality Improvement cohorts are currently underway. Eleven practices have chosen to do projects in the well-child visit space and six of them will be submitting for Maintenance of Certification (MOC). In addition to guiding the quality improvement projects, our Healthy Children and Women and Infants population health stream is working with several groups to improve care in our region. For example: The school-based health center in Xenia is working toward implementing electronic consents; The primary care practices affiliated with Dayton Children's are implementing outreach reports in Epic; The population health teams are working with the Dayton Children's urgent care sites to convert sport physicals to well visits; The team is also exploring new nursery partnerships. If you are interested in learning more about partnering with our population health teams, please contact PFKProRelations@childrensdayton.org.

Support Families Beyond the Exam Room
As a primary care clinician, you know that some families need support beyond what can be provided in the exam room. Our Care Guide team helps families understand their Medicaid coverage and re-enroll if needed, locate transportation services, access food assistance, find free back-to-school resources and stay on top of their appointments and medications. A child does not need to have a complex or chronic condition to work with one of our Care Guides.
Do you know a family who could benefit from this program?
