2026 Flu Clinic

Student Health Service is partnering with Dedrick's Pharmacy to bring flu shots to students and faculty on campus this fall.

Please see the attached Influenza Vaccine Information Statement and complete the attached Patient Information Sheet. Once completed, email your form and a photo of the front and back of your prescription insurance card to the Student Health Service. When we receive your information, we will call you to schedule your appointment!
