By listening, learning, and leading, ATTC’s Integration of Behavioral Health into Primary Care POWER Team is driving integrated care innovation. Here’s how:
Integrated Behavioral Health and Primary Care
Integrated behavioral health care is an emerging approach that combines medical and behavioral health services in a single setting. This model recognizes that both physical and mental health factors are essential to overall well-being. Medical and behavioral health clinicians work together as a team to address a patient’s concerns in recognition of the fact that both medical and behavioral health factors are important considerations for a person’s overall health. Care is typically delivered by these integrated teams in the primary care setting. An effective integrated care team has been defined as a group of multidisciplinary providers working together with the patient using a shared treatment approach and assisting the patient to achieve specific physical and behavioral health outcomes (Lardieri, Lasky, & Raney, 2014).
Essential elements of an effective integrated care team include:
- leadership and organizational commitment to the philosophy of integrated care;
-
team development involving selection of team members, clarity of roles and responsibilities, cross-training, and development of the operational support;
- a team process that clearly outlines how services will be provided, including consideration of how coordination and collaboration will be facilitated across disciplines; and
-
ongoing assessment to evaluate the effectiveness of the established model to inform adjustments to care and maximize impact (Lardieri, Lasky, & Raney, 2014).
